地域に根ざし、あらたに生まれ変わった整形外科クリニック

宮城県南部・角田市に位置する「まつもと整形外科リハビリテーションクリニック」は、2024年6月に開院した。
阿武隈川と台山公園に挟まれた一角にあり、敷地からは市のシンボルでもある「H-2ロケット」実物大模型を望むことができる。
先代が地域医療を支えてきたこの地で、長らく閉院していた旧クリニックを、息子である現院長が独立を機に新たなかたちで再生したものである。

地域には大規模病院が少なく、各診療所が生活圏を支える重要な拠点となっている。
院長からは、高齢者から学生まで幅広い世代が訪れる場として、「開かれた空間」と「快適な医療提供環境」の両立が求められた。


木造による軽やかさと、まちに開く建築を目指して

建物は木造2階建て、延床約140坪。1階をクリニック、北側2階を住居兼スタッフルームとする構成である。
旧建物はRC造で、2019年の台風時に床下浸水の被害を受け、西側擁壁の傾きも確認されていた。新築にあたっては、荷重を大幅に軽減できる木造を採用している。

外観はヒューマンスケールを意識したシンプルな箱型とし、縦方向のヴォリュームを抑えて周囲のまち並みに馴染むよう配慮した。
過度な造形的主張を避けながらも、明快な水平ラインと大開口が「まちに開く意志」を静かに表現している。

敷地の三方に患者・スタッフ・家族それぞれの出入口を設け、動線の交錯を避けつつ、適度な距離感を保った。
また、住居部分は外観上あえて目立たせず、クリニックとしての端正で落ち着いた印象をまち並みに与えている。


「開」と「閉」を織り交ぜた三層のゾーニング

クリニック部分は南北方向に3つのブロックで構成し、それぞれ明確な役割を持たせている。

南側ブロックは待合室とリハビリ室を中心に、白を基調とした全面ガラス張りの吹き抜け空間とし、明るさ・開放感・清潔感を実現。
FIX窓下の換気窓により自然換気も可能とし、室内環境の快適性を高めている。

中央ブロックには中待合を配置。低めの天井と暖色系照明により、患者が落ち着いて過ごせる心理的安心感を演出した。
また、退屈せず待てる工夫として、健康情報を発信するデジタルサイネージも設置している。

北側ブロックは診療室・処置室などの機能空間とし、プライバシーを重視。
最新のオープンMRIやX線装置を備え、機器が要求する構造・設備条件を満たしつつ、スタッフと患者双方の動線効率を最適化している。

南北のブロックが「開」と「閉」、「パブリック」と「プライベート」の対比をなし、中央の中待合がその緩衝帯として両者をやわらかく結びつけている。


住宅設計の知見を活かした高断熱・高性能建築

非住宅建築では断熱性能の確保が後回しになりがちだが、本建築では構造体レベルで性能を担保する設計を行った。

断熱材には、宮城県築館工場で製造される東北イノアック社製「サーマックス」を採用。
耐水性・耐火性・断熱性に優れ、ボード状で水に濡れても乾けば再使用可能という特性を持つ。阿武隈川流域の地勢上、水害リスクに対しても有効である。
開口部には樹脂サッシ＋トリプルガラスを採用し、UA値0.2台を実現。全館で温度ムラの少ない快適性と高い省エネルギー性能を確保した。
さらに、南面には庇と外付けブラインドを設け、夏季の日射遮蔽と冬季の日射取得を両立している。

住宅設計で培った知見を応用し、非住宅でありながら高断熱・快適環境を実現した点が、このクリニックの大きな特徴である。


高度医療と地域交流をつなぐ、まちの新たな拠点

明快なゾーニング計画、性能に裏打ちされた設計と施工、そして地方都市でありながら高度医療を可能にする最新機器の導入。
これらを兼ね備えた本クリニックには、開院当日、市内外から患者が押し寄せた
それだけ地域に待ち望まれた存在だったということだろう。

「まつもと整形外科リハビリテーションクリニック」が、今後も地元に寄り添いながら地域医療を支え、人々が安心して集う場として機能していくことを期待している。

A Community-Based Orthopedic Clinic Reborn

Located in Kakuda City in southern Miyagi Prefecture, Matsumoto Orthopedic Rehabilitation Clinic opened its doors in June 2024.
The clinic sits in a quiet corner between the Abukuma River and Taiyama Park, from which one can glimpse the city’s symbol — a full-scale model of the H-2 rocket standing in the park.
On this site, where the previous generation once supported local healthcare, a long-closed clinic has been reborn. The current director — the son of the original founder — rebuilt it as a new kind of medical facility upon establishing his own independent practice.

In this region, large hospitals are scarce, and each clinic plays a vital role in supporting everyday medical care within the community.
The director requested a design that could serve as a welcoming place for all generations — from the elderly to students — combining openness with comfort and functionality in medical service delivery.


Lightness of Timber, Architecture Open to the Town

The building is a two-story wooden structure with a total floor area of approximately 460 m².
The first floor houses the clinic, while the north wing of the second floor serves as a residence and staff area.
The former building was a reinforced-concrete structure that suffered underfloor flooding during a typhoon in 2019, along with a tilted west retaining wall. In rebuilding, the choice of timber construction significantly reduced the structural load on the site.

The exterior was designed as a simple, human-scaled box.
Vertical massing was kept low to harmonize with the surrounding townscape, while the clear horizontal lines and wide openings quietly express the building’s intent to “open itself to the community.”
Each of the three sides of the site is equipped with separate entrances for patients, staff, and the resident family, avoiding intersection of flows while maintaining appropriate spatial distance.
The residential portion is intentionally understated, allowing the façade to present a calm, dignified appearance as a civic building within the town.


A Three-Layer Zoning of “Open” and “Closed”

The clinic area is organized into three north–south zones, each with a clearly defined role.

The southern block, containing the waiting area and rehabilitation room, features a bright double-height space enclosed by full-height glazing in white tones — bringing in light, openness, and a sense of cleanliness.
Operable vents beneath the fixed windows enable natural ventilation, enhancing indoor comfort.

The central block houses the sub-waiting area.
With a slightly lower ceiling height and warm lighting, it provides patients with a calm, psychologically reassuring atmosphere.
Digital signage displays health-related information, offering visitors something engaging while they wait.

The northern block contains the examination and treatment rooms — functional areas designed for privacy and precision.
It is equipped with the latest open MRI and X-ray systems. The layout carefully accommodates the equipment’s structural and environmental requirements while maintaining smooth and efficient circulation for both staff and patients.

In this plan, the southern and northern blocks form a contrast of “open and closed,” “public and private,” while the central zone serves as a gentle intermediary that connects the two.


High-Performance Design Informed by Residential Expertise

In many non-residential buildings, thermal performance tends to be secondary to equipment-based comfort.
Here, however, the design ensures high performance through the structure itself.

The insulation material is Thermax by Tohoku Inoac, manufactured locally in Tsukidate, Miyagi Prefecture.
This board-type material offers excellent water, fire, and thermal resistance, and can be reused even after getting wet — a valuable feature given the site’s proximity to the flood-prone Abukuma River basin.
High-performance resin-framed triple-glazed windows were installed, achieving a UA value in the 0.2 range, ensuring both thermal uniformity and energy efficiency throughout the building.
On the south façade, eaves and external blinds control solar gain — blocking harsh summer sunlight while welcoming passive heat in winter.

Applying principles cultivated through residential design, the clinic achieves a highly insulated, comfortable environment rarely seen in small-scale medical facilities.


A New Hub Connecting Advanced Medicine and the Local Community

With its clearly defined zoning, high-performance structure, and state-of-the-art medical equipment enabling advanced care in a regional city, the clinic embodies a new standard for community medical architecture.
On the opening day, patients arrived not only from Kakuda but also from surrounding areas — a sign of how eagerly this facility had been awaited.

Matsumoto Orthopedic Rehabilitation Clinic now stands as both a trusted center for local healthcare and a place where people can gather with a sense of comfort and assurance — a new architectural presence deeply rooted in the life of the community.
